New Patient Registration
If you live within our Practice Area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof which could be by way of a recent utility bill which is within the last 6 months. We also require one form of photographic ID such as a passport or driving licence. If you have only recently entered the UK we will need to see your visa.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
Medical treatment is available from the date of registration. Please contact reception for further information.
Please come to reception and ask for an application form or print one from the link below and bring it with you. It would assist us if you are able to bring your Medical Cards with you.
The reception team will be happy to help you register, please come to the surgery after 9.00am and before 17.45pm with completed paperwork.
